You have the right to:
Receive considerate, respectful, and quality care in a clean and safe environment
Know by name the physician, nurses, and other staff members responsible for your care
Expect confidentiality of all information and records regarding your care
Receive an explanation of your bill for services rendered
Request a consultation from another provider
Give or refuse permission for care, treatment, or research
Be examined and treated in private
Plan and participate in your care
Receive care that supports your comfort and dignity
Have your family members and significant others treated respectfully
Take part in talking about ethical issues that arise in your care
Expect reasonable responses to your requests for services we provide
Receive complete information concerning your diagnosis and treatment in terms you can be reasonably expected to understand
You have the responsibility to:
Cooperate with your caregivers and follow the plan of care upon which you and your physician have agreed
Make known your understanding of the treatment plan
Express concerns or problems regarding your care
Provide complete and honest information about your health and lifestyle
Provide complete information about your health insurance and ensure payment for your care
Be considerate of others
Keep appointments or provide 24 hours notice of your intent to cancel
Allow 2 working days to process routine prescription refills, routine messages, and referral requests
Notify your physician of unexpected changes in your health
